The adenoma of a prostate - an adenomyomatosis of glands of a prostate at men is more senior than 50 years, causing obstruction of a target department of a bladder. The important role in a parentage of disease is played with an involution of a hormonal exchange in advanced age.
Signs, current. Disease arises after 50 years is more often, with the years frequency of disease increases - more than 50 % of patients are more senior than 60 years. In a clinical picture the dysuric phenomena prevail: a delay of the beginning of an emiction, weakening of a jet of urine, a pollakiuria caused by presence of a residual urine and its becoming infected. At substantial growth of a residual urine appear a pain above a pubis and desires on an emiction, then a pain in lumbar area (a vesicoureteral reflux); at a renal failure occurrence of gastrointestinal frustration is possible. Stages of disease. The stage 1 - preclinical - is more often observed in the age of 50-60 years on a background of the transferred infections of genitourinary system. There are insignificant disturbances of the certificate of the emiction, unpleasant sensations in a perineum, in the bottom of a stomach, a back department of the urinary channel. An initial sign can be a premature ejaculation, a hypospermia. Disease passes in 2 stage. Stage 2 - dysurias - the speeded up emiction in the beginning at night, and then and in the afternoon. Occurrence of a sign of an imperative emiction which unlike a cystitis is not accompanied by morbidity and opacification of urine is typical, but its intensity happens is very great. Periodically dysuric phenomena can disappear for acritical time. A joining infection strengthens a dysuria. There is a complicated emiction in the beginning after a dream, long sitting, overflow of a bladder. The jet weakens wet, especially in the beginning of the certificate of an emiction (the thin jet falls steeply downwards), the night pollakiuria and a polyuria amplifies. The general condition of the patient in this stage quite satisfactory. At an appreciable pollakiuria at night patients become nervous and irritable. The stage 3 - an incomplete chronic ischuria - is characterized by presence of a residual urine which quantity gradually accrues. The wall of a bubble becomes gradually more thin, there are numerous fine false diverticulums. Current of disease gradual, the patient gets used to the condition, not noticing enlarged quantity of a residual urine. It speaks also depression of sensitivity of a wall of a bladder. Gradually the bubble is stretched, it can contain up to 2 litres of urine. Pressure of the urine which has accumulated in a bladder overcomes resistance of sphincters, and urine starts to be allocated consensually on drops. A stage 4 - the period of "inconscience with overflow" - « detention with an incontience ». In this period the renal failure with the phenomena of an intoxication is expressed: thirst, growing thin, cardiovascular disturbances.
