The varicose varicocele - meets in the age of 16-35 years, as a rule, at the left. The varicose varicocele is the most widespread anatomic change at the men, suffering sterility (25 % in comparison from 10 % in the general population). According to modern representations, the varicose varicocele develops owing to disturbance of outflow from the left vein of a testicle which runs in renal under a right angle (on the right it runs into the bottom vena cava). The secondary (symptomatic) varicose varicocele can be consequence of a clottage. The bilateral lesion is caused by a primary incompetence of valves of a vein and presence of arteriovenous anastomoses. The raised pressure in a renal vein owing to this or that reason leads to a return current of a blood from a renal vein on a vein in a plexus of a testicle and further - on an external seed vein in the general ileal vein. Thus, the varicose varicocele is a part of the roundabout anastomosis developed owing to complicated outflow of a blood on a renal vein at narrowing of its lumen and consisting of a vein, a plexus and an external seed vein. The clump and stagnation of a blood lead to rise in temperature inside of a scrotum and damage a spermatogenesis.
Signs, current. Feeling of gravity, a pain in a testicle, a scrotum, on a course of the seed funicle, amplifying at an exercise stress and long standing. At a palpation of a scrotum veins of a seed funicle (« a ball of worms ») are defined. Current of disease gradually progressing. With normalization of a sexual life retrogress of signs is marked. Occurrence of a varicose varicocele in advanced age is more characteristic for a symptomatic genesis of disease. Stages of disease: 1 stage - is not present subjective sensations, the expanded veins are localized within the limits of a seed funicle and disappear in horizontal position; 2 stage - veins are dilated to the bottom pole of a testicle, gravity and a pain in the field of a testicle, the seed funicle becomes thicker; 3 stage - the expanded veins at the bottom of a scrotum below a testicle; a pain in a testicle, perineums, lumbar area, a sacrum. Disturbance of a spermatogenesis is marked.
